* Fee structure includes surface ablation procedures (PRK, LASEK, Epi-LASIK) as necessary and as offered by participating providers.

How the program works:

• Call QualSight toll-free at (877) 285-2010.
• QualSight will answer your questions, conduct a pre-screen for the procedure, and inform you of the closest provider locations.
• QualSight will collect a fully refundable $50 scheduling deposit or select a flexible financing option, which will waive the deposit requirement.
• QualSight will schedule the pre-operative exam over the phone.
• After the appointment has been scheduled, QualSight will send you a confirmation e-mail complete with the provider's contact information and directions to the office.
• Present your ID Card and confirmation e-mail at the provider's office on the day of your exam to confirm pricing.
• The provider will perform a comprehensive exam.
• If you are a candidate for the procedure, the provider will schedule a date to provide service.
• If you are not a candidate, Qualsight will refund your scheduling deposit if you paid one.
