Options Discount Program takes you in healthy directions by opening the door to discounts on a broad range of alternative therapies and wellness services.

Discount:

• Pre-operative exam
• The LASIK or PRK procedure
• Post-operative care
• Retreatment warranty

100% patient financing is available upon approved credit through TruVision.

Custom LASIK is now being offered in some locations for an additional fee.

How the program works:

1. Call TruVision toll-free at 866-339-2020.
   • Get answers to your questions and get pre-screened for the procedure.
2. If you pass the screening, TruVision will gather your information and allow you to choose a doctor based on your location.
3. TruVision will either collect a deposit (generally between $100-$150 per eye) for your exam or approve you for financing.
4. TruVision will schedule the pre-operative exam.
5. Present your ID card at the provider's office.
6. The selected provider performs a comprehensive exam.
   

7. The clinic will collect the balance prior to/or on the day of surgery if you are paying in cash. If you have financed the procedure through TruVision, the payment will be paid to the laser center automatically after the procedure has been performed.
8. The doctor performs the procedure and honors the retreatment warranty as long as the criteria below has been met.

Retreatment Warranty:

1. Written retreatment warranty offered to participating members.
2. TruVision contracted with surgeons and their facilities to waive all professional and facility fees in the event a retreatment is necessary.
3. The only cost not covered is the equipment manufacturer-licensing fee.
   • Depending on laser used, price could be $110 or $150 per eye
4. Retreatment offered for life so long as the member schedules an annual retreatment exam through TruVision with a participating provider.
   • The exam is currently $75.
5. Members will be reminded of the annual retreatment exam via mail or email.
